What to bring to your appointment:

• Completed patient forms (see patient form tab for these documents);
• Your insurance card(s);
• All current prescription bottles and over the counter medications;
• Pertinent information about your medical and surgical history; including any and all records from any other Urologist;
• All recent reports dictated from any Cat Scan, MRI, Biopsy, Surgical procedure, or X-Ray.
