What to bring to your appointment:

• Your insurance card(s);
• Referral from your Primary Care Physician, if necessary;
• All current prescription bottles and over the counter medications;
• Pertinent information about your medical and surgical history;
• Any recent x-rays or other scans on disc or film with dictated reports;
• Completed patient forms (see patient form tab for these documents).
