Female Forms

All new patients are required to complete ALL forms and arrive 30 minutes prior to scheduled appointment time. You must also bring any records pertaining to urology, cat scans, ultrasounds, lab work, all medications you are currently taking and pertinent records from your primary care doctor. If we do not have these records at the time of your appointment we will need to reschedule your appointment.

We cannot obtain these records for you.

Follow up patients who have not been seen in the last 6 months MUST also complete ALL forms and arrive 30 minutes prior to scheduled appointment time. Please bring a copy of any and all recent labs, cat scans, medications (ordered by anyone other than us) or emergency room records with you if you have had a recent ER visit and we did not see you. If you do not have these records we will need to reschedule your appointment.

Please bring all insurance cards and a picture ID to each appointment.
